Reservations in the sidebar does not work

In the tab of each activity, the form to book that is in the sidebar on the right does not work (it does nothing when you click on the field that should show you the available dates), just like if you click on the calendar below on an available date nothing happens either. However, in the calendar and the reservation that are not on the side, they do work correctly.

You use 2 booking forms in the same page in the lower left column and the upper right column, so the right form will not work: https://prnt.sc/JCTN3LDv4MVe
This booking form can only be used once on the singe booking object page, please delete 1 booking form on this page
