URGENT: Registration form disappeared from /my-account/ page

Registration form disappeared from http://ibnrushdbooks.com/my-account/ page, only login fields are displayed, also when click on the user icon and choose to create new account it redirects to http://ibnrushdbooks.com/wp-login.php?action=register.

Please resolve this issue as soon as possible, as the site will be live very soon.
